The Under Thimble is a stainless steel deflector worn on the under hand. It is attached with reusable adhesive pads. 8 pads are included and replacement pads are available (Ref 7168)

Best finger protector I've found for hand quilting
I'm a hand quilter, and have tried many, many ways to protect my underneath finger. This steel minithimble is now my preferred method - and I find doublesided sellotape is fine to keep it in place. Its small size makes it a pleasure to use, but easy to lose, and that's why I've had to buy again.
